Certificate of Business Fictitious Firm Name (DBA) for Clark County, Nevada

- Complete the client information form; then,
- On the first line of the "Certificate of Business Fictitious Firm Name" form, print the name of your existing Corporation or LLC.
- Complete the next (address) line. OK to use P.O. Box 70477, Las Vegas NV 89170 if you like.
- Then below that, print the name you wish to use as you DBA (doing business as). You may not use Inc., Corp., LLC, etc., unless that is the name of your existing company as filed with the Secretary of State of Nevada.
- Lastly, fill in at least one name, address, and signature area. Be sure to sign it!
- DBA acquisition fee is $60.00
- We will search the name for availability then submit your application. Please allow up to 2 weeks for return of your DBA. Expedited service is not available. DBA's must be renewed every 5 years.
