address to be used for mail receiving and forwarding services. If you would like our mail forwarding service click here)
LINE 3: CHQ will complete this line. (Or you can enter the same address as above.)
LINE 4: You must enter the latest date upon which the Partnership is to dissolve. (Enter a date, not a number of years.)
LINE 5: Name and address (your address, not ours) of at least the first initial General Partner. (You may acquire more Partners later.)
LINE 6: Name, address (your address, not ours), and signature of at least one organizer. (Can be the same as on line 5.) BE SURE TO SIGN THE FORM HERE!
LINE 7: CHQ will complete and endorse as your resident agent.
Be sure to include a separate sheet with your name, shipping/billing address and telephone/fax numbers.

Make one check payable to "Secretary of State" for $75.00
Make one check payable to "CHQ Incorporated" in the amount of $100.

For expedited formation (approximately 7 working days):
Make one check payable to "Secretary of State" for $200.00
Make one check payable to "CHQ Incorporated" in the amount of $100.

Provide payment to "Secretary of State" for $200.00 by credit card only.
