For an annual fee of $85., CHQ will become your new resident agent and we will never increase that fee. Each year, when you are due for renewal, we will mail you a statement reflecting Nevada's annual fee plus CHQ's fee of $85. You may make the transfer at any time. Transferring the agency is not connected with filing your annual "list". The transfer and "list" filings can be submitted separately. You are not required by the State of Nevada to notify your original agent of the transfer.
1. Click here for agent transfer forms, print the document, then press the BACK button on your browser.
2. On the "Certificate of Change of Agent" form, ENTER THE NAME OF
YOUR ENTITY on the first line (please print), THEN SIGN on "Signature of Officer" line.
Do NOT sign where it says "Authorized Signature of R.A. ...we will sign there.
CHQ will complete the form, and file the change with the State of Nevada.
3. Complete the "Client Information" page.
4. Use the credit card form, or make your check payable to: "CHQ Incorporated" in the amount of $145.00.
($85. for CHQ + $60. for state transfer fee.)
5. Mail to P.O. Box 70477, Las Vegas NV 89170, OR;
6. If paying by credit card, complete credit card and fax everything to 702 796-6694. (Secure fax line)
7. After approximately 2-3 weeks, you may confirm the transfer by clicking here, then searching you entity.

DO NOT SIGN THE FORM! CHQ will complete the form and return it to you.
NOTE: If you have prepared "articles" there will be an agent acceptance line at the bottom of the form. You may ignore instructions above and we will endorse your articles instead. Only one form of acceptance is required.
3. Complete the "Client Information" page.
4. Make your check payable to "CHQ Incorporated" in the amount of $85.00 or use included credit card form.
5. Mail back to PO Box 70477, Las Vegas, NV 89170 (or if using credit card) fax everything to 702 796-6694.
